THE USE OF POTASSIUM PERMANGANATE TO REMOVE NITRIC OXIDE FROM EXHALED GAS

California/OSHA standards restrict the amount of nitric oxide (NO) released into the work area in 8 hours to 25 ppm and the amount of nitrogen dioxide (NO_{2}) to 1 ppm^{1}. Use of potassium permanganate to scavenge exhaled gas has proven useful during transport of patients requiring NO therapy. At our facility we have tranported 6 neonatal patients on NO using this type of filter attached to a custom enclosed expiratory valve. This method has proven useful when a suction system that vents to the external environment is not available as may be the case during ambulance, air, or inter-hospital transport.

We evaluated the efficiency of Pura-fil^{(r)} in the removal of NO and NO_{2} from a certified Nitrogen/NO mixture. Pura-fil^{(r)} is a filter material used to remove contaminants from industrial sources. The media consists of aluminum oxide spheres coated with potasium permanganate. We constructed a filter using a 10 inch by 2 inch diameter acrylic tube with 15/22mm adapters sealed on each end. Polyster fiber material was used to keep the Pura-fil^{(r)} in place, and a removable cap allowed for refilling.To validate the effectiveness of the filter, we filled it with 200, 400, and 600 grams of media and analyzed for NO and NO_{2} at 3 therapeutic levels plus 800 ppm using a chemiluminescent analyzer (API, Inc). At 15 liters per minute and 800 ppm we measured the following:

No measurable amounts of NO or NO_{2} were released at 20, 40, or 80 ppm. We conclude that Pura-fil^(r) is a useful filter media for scavenging NO and NO_{2} from ventilator waste gas.
