Disease Management Across the Continuum

The best of times awaits the respiratory care practitioner who can think and work outside the box of the traditional acute-care-hospital paradigm. The goal of cost-effective medical care played through a vertically integrated health care model fits nicely with the concept of greater utilization of respiratory care services. This workshop presents a model for such a "disease management" approach for the patient with COPD, highlighting primary and secondary prevention, acute intervention, post acute care in the skilled nursing facility, home, physician's office, and other sites. In each of these areas, new vistas of potential RCP involvement are explored. A management strategy for care delivery in most of these sites is provided, stressing outcomes and management through automation.

At the conclusion of this workshop, attendees will be able to: * understand the concept of disease management; * know the model of disease management that begins with "primary & secondary prevention," spans across the continuum through acute and subacute care, and ends with "post care follow-up"; * understand the concept and the need to develop care maps throughout this continuum (including ER, acute care, long-term acute, subacute, and home health); and * understand the need for outcomes studies "across the continuum."
