A NEW METHOD TO CHARACTERIZE THE EFFECTS OF SPACERS AND HOLDING CHAMBERS ON POOR HAND-BREATH COORDINATION DURING MDI USE?

Background: Spacers and holding chambers are recommended to minimize the common clinical problem of poor hand-breath coordination (dysynchrony) while using metered-dose inhalers (MDIs), however their ability to decrease the effects of dysynchrony by design have not been differentiated. Methods: In a model that simulated spontaneous respiration, we tested albuterol deposition from 9 devices in combination with a MDI, when actuation was or was not synchronized with inspiration. We characterized the devices as shown below. Results: With actuation 1 Sec before inspiration, spacers and all HCs delivered more drug than the MDI alone or low volume spacers(p < 0.001). During exhalation, more albuterol was delivered with valved-HCs than Spacers or MDI alone (p < 0.0001).

Conclusion: Valved holding chambers should be recommended for patients with poor hand-breath coordination while using MDIs. VA Research Service
