Weaning in the Intensive Unit: Predictors in Strategies
James K. Stoller, M.D.
This presentation reviews aspects of weaning patients from mechanical ventilation. Specifically, the diagnostic performance of both available univariate and multivariate weaning predictors will be presented. The discussion will focus on the predictive performance of various weaning "parameters", emphasizing their value in predicting weaning success and failure.
The presentation will then review available strategies for weaning and will review available literature regarding the efficacy of different approaches (e.g., T-piece trials, IMV, and pressure support).
Finally, the presentation will review additional considerations in weaning from mechanical ventilation, e.g., addressing ischemia, optimizing psychologic readiness, and offsetting autoPEEP.
