MECHANICAL VENTILATION IN PATIENTS WITH ORGANOPHOSPHORUS POISONING: A STUDY OF CLINICAL OUTCOME

BACKGROUND
Organophosphorus poisoning is a problem of the third world countries. This insecticide is freely available and used commonly to disinfect the crops. Both accidental and suicidal ingestion are common in India and in sufficient doses lead to respiratory failure. The present study was done to evaluate the need for mechanical ventilation and its overall outcome in patients with organophosphorus poisoning.

METHOD
This is a retrospective and epidemiological study of patients with organophosphorus poisoning admitted to Kasturba Medical College Hospital, Manipal from 1.1.2005 to 31.12.2005. The case records of all these patients were studied with respect to the use of pralidoxime, atropine or glycopyrrolate, incidence and duration of mechanical ventilation, ICU stay and outcome. A total of 68 patients could be followed up.

RESULTS
The average age (mean ± SD) of the patients was 29.8824 ± 8.4244 years. There were 54 males and 14 female patients.

A total of 51 patients (75%) were ventilated. The patients were ventilated for an average of 5.3971 ± 4.2149 days (mean ± SD).  17 patients were not ventilated(25%) 

The patients’ ICU stay was on an average of 7.7794 ± 4.1783 days (mean ± SD).

Thirty two patients (47%) were administered pralidoxime. In these patients, the mean (SD) duration of treatment was 0.8676 ( ±0.9759 ) days.

Thirty six (52%) patients were given glycopyrrolate. The mean (SD) duration of treatment was 1.3235 ± 1.7315 days.

Fifty four (79%) patients received atropine. The mean (SD) duration of treatment with atropine was 5.2206 ± 4.1747 days.

Fifty four patients (47%) were administered atropine. In these patients, the mean (SD) dose administered was 6.9412 (6.3478) days

Thirty two patients (47%) were administered pralidoxime. In these patients, the mean (SD) dose administered was 0.4706 (0.5028) days
The overall mortality rate was low [11/68 (16%)]. 11 patients were on ventilator and 9 patients died due to cardiac arrest. 2 were declared discharge against medical advice (DAMA).

CONCLUSION
Most patients with organophosphorus poisoning belong to young males (15 – 30 years). Most patients (77 %) require mechanical ventilation.

The overall mortality rate was low [11/68 (16%)]. 11 patients were on ventilator and 9 patients died due to cardiac arrest and 2 patients were discharged against medical advice.
