INTRODUCTION: The 7 adult ICUs at UMMHC and the initiation of VISICU/Ecare manager, a telemedicine and clinical management system or electronic ICU, has provided our ICUs with a unique multi-professional care plan improving patient care, safety and clinical outcomes.
Method: Various strategies have been undertaken to improve the availability of critical information. In Massachusetts, UMMHC is the only institution and one of seventy nationwide to offer this electronic ICU 24/7. This project complete with a central command center has provided multiple proactive improvements in the care of our patients.
* Length of stay
* Average vent days
* Median vent days
* Blood stream infections
* Glycemic control
* Ventilator weaning compliance
These clinical outcome measurements before initiating the eICU ( 10/05 to 6/06 ) were compared to the outcomes after initiation of the eICU ( 7/06 to 12/06 )
* LOS decreased by 5%
* Mortality rates decreased by 14%
* Average ventilator days decreased by 4%
* Median ventilator days decreased by 15%
* BSI decreased by 3.3%
* Glycemic control achieved 13% improvement of pts
* 99.7% adherence to ventilator weaning guidelines
* There were over 6,600 interventions by the eICU
After 6 months of using an electronic ICU to provide both a multi-professional care plan and continuous attending coverage of our adult ICUs we demonstrated a significant improvement in several critical ICU benchmarks, and a significant cost savings to the institution. The eICU offers benefits to the physicians, nurses, and respiratory therapists by providing information, patient profiles, and tools that support bedside care. The multiple interfaces and integrations between the eICU and the patient has provided superior quality of care.
