LAB EVALUATION OF THE INOMAX DS IN BACKUP MODE, JOHN NEWHART RRT RCP, UCSD MEDICAL CENTER, SAN DIEGO CA.

Background: Since 1997 the INOvent has been the device of choice by INOTherapeutics for delivery of inhaled nitric oxide (INO) to patients. A newer updated device called the INOmax DS is in the final process of testing prior to release. One of the features of the new system is a mode called “backup mode”. This mode has a variety of uses including emergency use. This mode delivers 800ppm NO at a non-adjustable flow of 250mL/min. The purpose of this study is to demonstrate the effect on NO concentration delivered to the patient of different ventilator parameters particularly with differing minute volumes (Ve).

Methods:
A Viasys Pulmonetic Systems LTV 1000 was used in the testing. The LTV has a pre set bias flow of 10 Lpm that was assumed to be consistent. The ventilator was set to a variety of tidal volumes and respiratory rates. Ve was captured from the LTV display and recorded along with the PPM NO concentration displayed on the INOmax DS. While Vt, RR and NO measured were recorded only Ve was compared to NO. This was done as Ve had the best correlation of the ventilator setting variables.
Results:

Conclusion: Measured NO values decrease proportionately with an increase in Ve. Inconsistencies can most likely be explained by variations in other variable ventilator parameters including RR, flow rate, and tidal volume.
Discussion: This study did not consider the impact of several ventilator setting variables. However in spite of that a clear correlation between Ve and measured NO is apparent. This comparison can help demonstrate the relationship of Ve to measured NO when using the Backup mode on the INOmaxDS when using the LTV. Testing continues using other ventilators.
