Background: INSTITUTION OF A SMOKING CESSATION PROTOCOL FOR PATIENT EDUCATION. Utilizing the evidence based 5 A- Model to provide successful patient interventions and referral for follow up after discharge.
Method: Staff was trained by a grant funded tobacco education program offered by the state to teach the ASK, ADVISE, ASSESS, ASSIST and ARRANGE Model to provide bedside interventions to the patients. The hospitalâs pharmacists provide recommendations for nicotine replacement treatment. Patients are given the opportunity to have a consent referral for follow up including phone counseling, class arrangement, and nicotine replacement discount.
Results: From November 1, 2006 to March 31, 2007 the Respiratory Department provided 268 smoking cessation interventions with 100 consent referrals to the outside agency for follow up. The referral percentage for follow up care outside the hospital was 37%. Of the referrals, 69% were reached by phone from the outside agency. Further follow up from the outside agency is broken down in the attached table.
Conclusion: The protocol has been a successful tool in providing patients with education and potential follow up for outside the hospital setting. By implementing a mandatory training to staff in basic brief tobacco interventions, it has proved to provide them the skills necessary for non-judgmental and supportive tobacco cessation education to patients, which has increased the patients' knowledge and outcomes.
